Dr Vipul patadia, MD is LIC Medical Examiner & Cardiologist

• Medical & ECG Examination is done during OPD hours (office Consultation ).

• LIC agent must accompany with proposed life for introduction.

• Identification proof of Agent as well as proposed life is mandatory.

• Examination Limit is Rs 9,99,999

• LIC agents are requested to co-operate nursing & reception staff during procedure & protocol.

(4) For Physician Report, Questionary form, Deformity form please bring FMR , ECG, Lab report,X-ray , USG report & proposal form with you.
